Completion of this form is a request only and does not guarantee an appearance. Request forms must be received at least three weeks prior to the event date for a request to be considered.

There is an appearance fee of $100. For events away from UBC's Vancouver Campus, it is the responsibility of the booking group to provide or pay for travel.
